Based on these inputs, the calculator would first determine the billable weight. The actual weight is 10 lbs. The dimensional weight would be (12 * 10 * 8) / 139 = 6.9 lbs. Since 10 lbs (actual) is greater than 6.9 lbs (dimensional), the billable weight is 10 lbs.
Given the cross-country distance, a base cost and a per-pound rate for Ground service are applied, both adjusted by a distance multiplier. These are summed to get a subtotal. A fuel surcharge (e.g., 5%) is then applied to this subtotal. Since the declared value is $100, there's no additional declared value surcharge in this specific example (as it's at the threshold).
