CHADS-VASc Score Calculator

The CHADS-VASc score is a clinical prediction rule for estimating the risk of stroke in patients with non-valvular atrial fibrillation (AFib). It helps clinicians decide whether a patient should be on anticoagulation therapy to prevent stroke. A higher score indicates a greater risk of stroke and generally warrants anticoagulation.

Understanding the CHADS-VASc Criteria

The acronym CHADS-VASc stands for specific risk factors, each assigned a certain number of points:

  • Congestive Heart Failure/LV Dysfunction: 1 point
  • Hypertension: 1 point
  • Age ≥ 75 years: 2 points
  • Diabetes Mellitus: 1 point
  • Stroke/TIA/Thromboembolism: 2 points
  • Vascular Disease (e.g., prior myocardial infarction, peripheral artery disease, aortic plaque): 1 point
  • Age 65-74 years: 1 point
  • Sex Category (Female): 1 point

The maximum possible score is 9. Based on the total score, a patient's stroke risk can be stratified, guiding treatment decisions.

How to Use the Calculator

Simply enter the patient's age and check all applicable risk factors below. The calculator will automatically compute the CHADS-VASc score.

Interpreting Your CHADS-VASc Score

The calculated score helps in risk stratification:

  • Score 0 (Males) / 1 (Females): Low risk. Anticoagulation may not be necessary.
  • Score 1 (Males) / 2 (Females): Moderate risk. Consider oral anticoagulation.
  • Score ≥ 2 (Males) / ≥ 3 (Females): High risk. Oral anticoagulation is strongly recommended.

It's crucial to remember that the CHADS-VASc score is a tool to aid clinical decision-making and should always be used in conjunction with a comprehensive patient assessment by a qualified healthcare professional. Individual patient factors, bleeding risk, and patient preferences must also be considered.

Example Scenarios:

Example 1: A 70-year-old female with hypertension and diabetes.

  • Age 65-74: 1 point
  • Female Sex: 1 point
  • Hypertension: 1 point
  • Diabetes Mellitus: 1 point
  • Total CHADS-VASc Score: 4 points (High risk, anticoagulation strongly recommended)

Example 2: A 55-year-old male with no other risk factors.

  • All factors negative.
  • Total CHADS-VASc Score: 0 points (Low risk, anticoagulation likely not needed)

Example 3: An 82-year-old male with a history of prior stroke and congestive heart failure.

  • Age ≥ 75: 2 points
  • Prior Stroke: 2 points
  • Congestive Heart Failure: 1 point
  • Total CHADS-VASc Score: 5 points (High risk, anticoagulation strongly recommended)
