Understanding GLP-1 Dosage Calculations
Glucagon-like peptide-1 (GLP-1) receptor agonists, such as Semaglutide and Tirzepatide, are highly effective medications for weight management and type 2 diabetes. However, because these medications are often titrated (increased gradually over time), patients must understand how to calculate the correct volume for injection, especially when using compounded versions or multi-dose vials.
How to Calculate Injection Volume (mL)
To determine how much fluid to draw into your syringe, you need to know two variables: your Target Dose in milligrams (mg) and the Concentration of the medication (mg per mL). The formula is:
Example: If your doctor prescribes a 0.5 mg dose and your vial has a concentration of 5 mg/mL, the math is 0.5 รท 5 = 0.1 mL. If you are using a standard U-100 insulin syringe, 0.1 mL is equivalent to 10 units.
Tracking Your Weight Loss Progress
Success with GLP-1 medications is often measured by the percentage of total body weight lost rather than just the number on the scale. Clinical trials for Wegovy and Zepbound typically report average weight losses ranging from 15% to 22% of starting body weight over a 68-week period.
Using the percentage formula helps you visualize your health improvements, as even a 5% to 10% reduction in body weight is associated with significant metabolic health benefits, including improved blood pressure and cholesterol levels.
Important Titration Schedules
Most GLP-1 protocols follow a standard titration schedule to minimize gastrointestinal side effects like nausea or reflux:
- Month 1: 0.25 mg (Semaglutide) or 2.5 mg (Tirzepatide)
- Month 2: 0.5 mg (Semaglutide) or 5.0 mg (Tirzepatide)
- Month 3: 1.0 mg (Semaglutide) or 7.5 mg (Tirzepatide)
